Minimum Requirements:

• Current 2 years of current Physical Therapist experience
• Current ID Physical Therapist License
• Current AHA BLS
• Skills Checklist from within the past 12 months
• Drivers License
• EMR Experience (Include years of experience, and systems used)
• Final RTO or any scheduling requests
• Two Supervisory References from the past two years of employment
• Healthtrust Cover Sheet
