Type: Occupational Therapist     

Location: Memphis, TN      

Fidé is currently seeking an Occupational Therapist for a 13-week contract in Memphis, TN.     

Minimum Requirements: The following experience is required at least two years of current Occupational Therapy experience. The Occupational Therapist plans, organizes, develops, administers, directs, and supervises OT treatment in accordance with current applicable federal, state, and local standards, regulations and guidelines. Promotes Patient and Employee Safety – Focused on 10 Foot Circle of Safety and displays safety awareness in all of the below key areas:
• Safe Patient Handling
• Materials Handling
• Infectious Diseases
• Slips/Trips and Falls
• Work Environment

Follows all facility guidelines outlining standards of personal appearance, attendance and punctuality.

Assesses patient’s level of functioning upon physician orders. Plans and implements the appropriate treatment based upon assessment.
Participates in the interdisciplinary team process.
Participates in discharge planning for each patient including placement, patient/family education and adaptive equipment.
Completes appropriate documentation according to Department policy and procedures.
Supervises Certified Occupational Therapy Assistants. Supervises OT and COTA students in the application of theory and techniques of Occupational Therapy. Assists with new staff orientation. Supervises students or other staff members competently.
Displays a “customer service” attitude toward patients, visitors, and other staff members at all times.
Promotes an ethic of continuous quality improvement in all matters associated with the position, and documents specific quality assurance implementation.
Is effective, efficient and appropriate in evaluating each patient. Completes initial evaluation in the patient’s chart according to policy. Evaluates appropriate areas based on the patient’s diagnosis and information gathered from the medical record. Selects and accurately administers appropriate evaluation procedures.
Bases individualized treatment plan on results of the evaluation process along with patient and family stated goals.
Seeks out resource materials and utilizes fellow staff members for treatment ideas and techniques appropriate to the patient.
Communicates with patients and/or family members at their level of understanding.
Shows initiative to collaborate with other team members to provide the best patient care.
Completes proper coverage information for scheduled absences.
Completes all necessary documentation. Completes daily progress notes within defined timeframe. Written independent treatment program and/or family caregiver training recommendations provided as appropriate and documented when not appropriate.
Maintains flexibility in scheduling patients and is cooperative in resolving conflicts.
Shows an interest and understanding of importance for all scheduled conferences. Attends team conferences. Provides pertinent information and collaborates with team members in an appropriate manner.
Goes through all the appropriate steps in the discharge planning process. Properly issues or recommends equipment to patient/family member and documents it when appropriate. Documents training and use of equipment with patient and/or family. Documents discharge disposition. Reflects discharge plan in treatment plan. Discusses discharge plans at staff meetings, with patient and/or family, and at family conferences.
Demonstrates good safety techniques regarding patient, self, others, and equipment.
Observes patient confidentiality.
Instills confidence in team members, patients, and families.
Budgets time and sets priorities.
Performs all other duties as assigned.


Hours Per Week: 40
Shift: Days 8 hr shifts 5 days per week
